Amended Nonresident and Part-Year Resident Income Tax Return
Form IT-203-X may be used by part-year residents and nonresidents who want to amend their New York income tax return. To complete New York Form IT-203-X, the taxpayer will need additional forms such as federal Form 1040 (Schedule C or C-EZ, D, F), federal Form 4797, Form IT-203-ATT, and Form IT-370.
Amended Resident Income Tax Return
Form IT-201-X can be filed only by residents who want to amend their New York income tax return. To complete New York Form IT-201-X, the taxpayer will need additional tax forms such as federal Form 1040 (Schedule C or C-EZ, D, F ), Form Y-203, Form IT-360, and federal Form 4797.
New York Resident Income Tax Return for Full-Year State Residents Only (Tax Booklet)
The New York tax booklet is a guide for state residents on how to file their state income taxes. The guide features step-by-step instructions on how to file by mail or e-file your state taxes. In addition, the booklet features information on every type of state tax form as well as areas where filers commonly make mistake.
New York Depreciation Adjustment Schedule
Form CT-399 may be filed by corporations who are eligible for depreciation adjustments. To be able to complete and submit NY Form CT-399, corporations will need to complete federal Form 4626 first.
NY Form CT-399 must be filed as an attachment to federal Form 4562.
New York Application for Automatic Six-Month Extension of Time to File for Individuals
The NY IT-370 form is used for New York residents who seek an extension of the filing deadline for their state income tax returns. Successfully filing the NY IT-370 automatically allows New York residents a six month extension to file their NY IT-201 or NY IT-203 state income tax returns.
New York Resident Income Tax Return for Full-Year State Residents Only (Fill-in & Save)
Form IT-201 can be used only by resident New York taxpayers who want to file their New York income tax returns.If you are a part-year resident or a nonresident, you may use Form IT-203 instead to file your income tax return.

New York Individual Part-Year and Non-resident Income Tax Return
Form IT-203 is the form all New York part-year residents and nonresidents need to use to file their New York tax returns. Although Form IT-203 may be supplemented by additional forms or schedules, it must be included in all New York nonresident and part-year resident returns.
New York Individual Resident Income Tax Return (Short)
New York form IT-150 is designed for state residents to report their annual income. The form features space to include your income, marital status as well as space to report any children or dependents you may have. By filling out the form, you will see whether or not you owe money to the state or if you will receive a refund.
Resident Income Tax Return - Form IT-150 was discontinued. All full-year residents must file Form IT-201
New York State Resident Credit
Form IT-112-R is a supplemental form and as such should be filed with either Form IT-150 or IT-201. Form IT-112-R prevents New York residents who work in another state from facing dual taxation on income earned in other states. Without Form IT-112-R residents who work in another state would face dual taxation because they would be taxed on income that they earned outside of New York (their state or residence) and the other state (where they earned the income). Form IT-112-R prevents such dual taxation by issuing a credit for taxes paid to other states.
New York Noncustodial Parent Earned Income Credit
Tax form IT-209 is for use by New York State residents who want to claim the noncustodial parent earned income tax credit. The filer can claim either the NYS earned income credit, or the NYS noncustodial parent earned income tax credit, but not both. To qualify for the credit, during the previous tax year one must have: resided in New York State, been 18 years of age or older, and had minor children who resided elsewhere. Several other conditions must also be met to qualify.
New York Summary of Federal Form 1099-R Statements
Form IT-1099-R must be filed by all taxpayers who received a 1099-R statement which shows the NYC withholding.
This form has been discontinued.
New York Empire State Child Credit
Form IT-213 can be used by full-year residents to claim an Empire State Child credit. A full-year resident, who wants to apply to receive Empire State Child credit, must complete and mail NY Form IT-213 with their New York income tax return.
New York College Tuition Credit
Form IT-272 can only be used by taxpayers who are residents and want to claim a college tuition credit or want to register for deductions. New York Form IT-272 must be completed and mailed with one’s income tax return.
Non-residents of Yonkers Who Earned Income In Yonkers
Form Y-203 can be used by nonresidents who want to report their earned income from a New York source. To complete New York Form Y-203, you will need additional forms such as Form IT-201 or Form IT-203.
New York Child and Dependent Care Credit
Form IT-216 may be used by eligible taxpayers to claim a credit for Child and Dependent care.

New York Unemployment Compensation Payments
Form IT-1099-UI can be used by taxpayers who are required to report their tax returns. You can file NY Form IT-1099-UI, only if 1099-G form was issued by the NYS Department of Labor and shows your NYS withholding.
This form has been discontinued.
New York Earned Income Credit
Form IT-215 may be used by taxpayers who want to claim a credit on their earned income.
To complete New York Form IT-215, the taxpayer will need additional forms such as federal Form 1040, federal Form 1040EZ, New York Form IT-201, Form IT-203-ATT, and Form IT-203.
Form IT-215 must be completed and mailed together with Form-203 or Form-201.
New York Real Property Tax Credit for Homeowners and Renters
Form IT-214 may be used by renters and homeowners who want to claim a Real Property Tax Credit. If you file this claim with your New York income tax return, you will need to file Form IT-201 as well.
